[Q] [Help] Restore Script For Lollipop 5.0.2 topic

About Two months ago I downloaded this restore script shown in the image.....
The problem is that I forgot the thread I downloaded it from and also unable to find that thread again XD :p. Can anyone tell me the instructions to run this auto restore script so that I can install stock lollipop again on my phone.
The build no. and all the stuff matches perfectly with the stock firmware I had before.:good:
