Phone app FC when doing conference call topic

SOCIALIZE IT ⇨


I've read that folk had com.android.phone crashing but nothing related to my situation.
ROM - KK, rooted, Xposed, blocked lot of crap and spying stuff.
I can call people, pause and resume call, but when want add third person - "application stopped working".
It worked some weeks ago, now it doesn't.
Surely I did something but no way to figure out what.
I wiped dalvik but really don't want go through factory reset.

I caught logcat info, attached in file.
If anyone who can analyze such thing would have a look and give a hilt - I'd be grateful.
